Employee Scrubs Requirements
• You are required to purchase 2 pairs of personalized American Laser Med Spa scrubs within 60 days of start date.
One Black & One Royal Blue pair
• Within 30 days of your start date – Order your 1st pair (Black)
• Within 60 days of your start date – Order your 2nd pair (Royal Blue)

IMPORTANT – Make sure all information is accurate. Especially the spelling of your name.

Payment & Reimbursement
You are responsible to pay Stacy’s Uniforms when you place the order.
Cost is approx $88 per pair and $176 for 2 pairs
You will be reimbursed $200 for your scrubs on your one-year work anniversary
Regular Sizes = Top $33 Bottom $37 Embroidery $17 = Total $88 per pair. $176 for 2 pairs
Large (sixe 2x and beyond) – Top $37 Bottom $42 Embroidery $17 = Total $96 per pair. $192 for 2 pairs

Dress Code
When wearing the scrubs, they should be free of wrinkles, frayed edges and clean with no stains.
They should be proper length, not above the ankle or drag on the floor and should fit properly, not overly baggy or too tight.
All personnel performing procedures or working in the Clinic MUST wear matching scrubs.
Corporate office employees are not required to wear the scrubs, but they do have the option to order them if they would like.
Shoes
We are currently in the process of selecting a new pair of shoes as the last pair have been discontinued. Once selected they will also be a requirement when wearing the scrubs.

Clinic employees are required to order both.
Corporate employees are given the option to order both but are not required.
2. When will scrubs and shoes be ordered for new employees?
Within 30 days of hire date order 1st pair and within 60 days of hire order the 2nd pair. Fill the form on intranet. HR will help coordinate
3. Is there a minimum requirement for the number of scrubs an employee should have?
Yes, 2 sets. 1 Black and 1 Royal Blue.
4. What is the cost to employees for the scrubs and shoes?
Top (Regular sizes) = $33 & Top size 2X and beyond = $37
Bottom (Regular sizes) = $38 & Bottom size 2x and beyond $42
Embroidery Fee $17 & Embroidery Fee $17
TOTAL COST PER REGULAR PAIR $88.00 = Total for 2 sets = $176 &
LARGE SIZES COST IS $96.00 PER PAIR (prices subject to change) = Total for 2 sets = $192
5. When will Business Cards be ordered for new employees & what’s the cost?
Within 60 days of employment. Cost – approx. $20. Contact HR for details

-Full Name of Employee
-Position of Employee (Laser Tech, Clinic Manager, Sales)
-Company Name
8. How will the Employee pay for the scrubs and shoes?
New employee will pay Stacy’s Uniforms directly
9. Will the Employee be reimbursed for purchasing scrubs and shoes?
Yes. Upon reaching your 1-year anniversary with our company Employees will be reimbursed $200.00. In other words, if you’re with the company for 1 year – your scrubs and shoes are on us!
