Eligible employees are those that work at least 36 hours per week on a full time basis.
Plan change and dependent additions that are chosen after the initial selection without a qualifying event are allowed only during open enrollment.
Open enrollment each year is the month of May for the effective date of June 1st each year.
All newly hired employees will be eligible for coverage at the first of the month after 60 days of employment.
The premium is based on a 26 bi-weekly pay periods.
Employee to contact Human Resources/Christine Gallegos when they become eligible to get information about insurance and premium costs. The premium cost is age based and will be different for employees.
The employee will have the option to add his/her children or spouse to the plan as well.
